Understanding Keratoconus: Stages, Symptoms & Modern Management
Keratoconus is a progressive eye condition in which the normally round cornea thins and bulges into a cone-like shape. This irregular curvature distorts light entering the eye, leading to blurred vision, glare, and increasing dependence on specialty contact lenses or surgical intervention.
What Causes Keratoconus?
The exact cause is not fully understood, but research points to a combination of genetic predisposition and environmental factors. Common associations include:
- Family history — first-degree relatives have a significantly higher risk
- Eye rubbing — chronic mechanical trauma can accelerate corneal thinning
- Atopic conditions — eczema, asthma, and allergies are frequently linked
- Connective tissue disorders — such as Ehlers-Danlos syndrome or Marfan syndrome
Keratoconus typically begins in the teenage years and progresses through the twenties and thirties, though it can stabilize at any stage.
Recognizing the Symptoms
Early keratoconus is often mistaken for simple refractive error. Watch for these signs:

Management Options

Spectacle Correction
In very early stages, glasses may provide adequate vision. As the cornea becomes more irregular, spectacle correction alone is usually insufficient.
Specialty Contact Lenses
This is the cornerstone of keratoconus management for most patients:
- Rigid gas permeable (RGP) lenses vault over the irregular cornea
- Hybrid lenses combine a rigid center with a soft skirt for comfort
- Scleral lenses rest on the sclera and vault entirely over the cornea, providing exceptional optics and comfort for moderate to advanced disease
Corneal Cross-Linking (CXL)
When progression is documented, corneal cross-linking strengthens corneal collagen fibers using riboflavin and UV light. CXL does not reverse existing damage but can halt further thinning — making early referral critical.
Surgical Options
In advanced cases where contact lenses are no longer tolerated, intracorneal ring segments (ICRS) or corneal transplantation may be considered. These are typically last-resort options after exhaustive contact lens management.
